Insurance
Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking the following questions:
- Do I have mental health insurance benefits?
- What is my deductible and has it been met?
- How many sessions per year does my health insurance cover?
- What is the coverage amount per therapy session?
- Is approval required from my primary care physician?
Payment
I accept Medical Mutual, The Ohio State University, and Anthem/ Blue Cross Blue Shield. I also accept Health Savings Accounts, cash, check, and all major credit cards.
Financial Issues
The payment of your bill is considered part of your treatment. If you wish, my office will be happy to file insurance for you. All you have to do is provide me with your insurance card and my billing staff will do the rest.
Fees
I charge was is usual and customary for our area. I charge was is usual and customary for our area. My fee is $130-$140. Insurance often helps. Half hour appointments at half cost may also be scheduled depending on the circumstances. I take credit cards, cash, check and HSA cards.
Insurance
For those insurance plans where I am a participating provider, all co-pays and deductibles are due at the time of treatment. I expect those payments at the time of service. You are responsible for knowing the provisions of your insurance and for advising me regarding the terms of your policy for mental health treatment. This information can be onbtained from your insurance company by checking on line or asking them the following questions:
Am I recognized provider for this policy? If not, does your policy pay “Out of Network” benefits?
What is the amount and status of your annual deductible, if you have one?
What is the amount of your co-pay, if any? (Note this is a “split” between what your insurance pays and what you pay and is either a set amount of a percentage of my hourly fee).
How many visits does your policy allow per year?
Cancellation Policy
Appointments are 50-60 minutes in duration. You should expect to wait no more than 10 minutes for your scheduled appointment. You are also expected to notify this office 24 hours in advance if you need to reschedule or cancel an appointment. My voicemail can be reached 24 hours a day/ 7 days a week. Missed Appointments
It is my policy to charge 1/2 my customary charge for missed appointments that are not cancelled 24 hours prior to the scheduled appointment time. This is not covered by insurance. Please help me serve you better by keeping your scheduled appointments or by notifying me in advance if changes need to be made.
